Job Summary:
Provide psychosocial assessments, supportive therapy, and discharge planning to clients. Collaborate with multidisciplinary teams to ensure comprehensive patient care and make appropriate referrals as needed.

Responsibilities:

• Conduct psychosocial assessments to evaluate client needs and recommend appropriate interventions.
• Deliver supportive therapy sessions to assist clients in coping with emotional, behavioral, and mental health issues.
• Develop and implement discharge plans that include referrals to community resources or additional services.
• Document clinical findings and treatment plans in accordance with organizational standards.
• Maintain compliance with licensure requirements and ensure continual professional development.

Qualifications:

• Hold a Master’s Degree in Social Work from an accredited institution.
• Possess a valid LMSW license issued by NYSED, with primary source verification.
• Demonstrate at least one year of LMSW clinical experience.
• Exhibit strong interpersonal, communication, and assessment skills.
• Ability to work independently and as part of a multidisciplinary team.
