Job Summary:
Provides specialized speech and language therapy services to patients in their homes, assessing communication, swallowing, and cognitive abilities to develop individualized treatment plans. Collaborates with healthcare teams, patients, and families to maximize functional recovery and independence.

Responsibilities:

• Perform comprehensive patient assessments to determine appropriateness for therapy and establish baseline function.
• Develop and implement evidence-based treatment plans targeting communication, swallowing, and cognitive skills.
• Instruct patients and families on therapeutic exercises and strategies to improve communication and safe swallow function.
• Document progress, revise care plans as necessary, and ensure compliance with agency policies and regulatory standards.
• Communicate regularly with physicians, nurses, and interdisciplinary team members to coordinate care and adjust therapy approaches.
• Educate patients and families using age-appropriate teaching methods and evaluate the effectiveness of educational tools.

Qualifications:

• Hold a Master’s degree in Speech-Language Pathology from an accredited program.
• Possess a current NJ Speech-Language Pathologist license.
• Have at least one year of experience in acute, subacute, or home health settings.
• Demonstrate strong customer service and communication skills.
• Show knowledge of home care regulations, insurance reimbursement, and documentation standards.
