Job Summary:

Seeking experienced Registered Nurses to conduct comprehensive assessments in home health settings, supporting enrollment and care planning for diverse patient populations. Candidates must be skilled in evaluating health status, documentation, and care coordination, working both in-field and via telehealth modalities.

Responsibilities:

• Perform initial in-home pre-evaluations assessing health, strengths, and care needs to develop individualized long-term care plans.
• Review medical documentation and health referral forms to ensure accurate information gathering.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns, conflicts, or issues identified during assessments to support care continuity.
• Communicate health care options, support services, and vendor choices with consumers.
• Collaborate with interdisciplinary teams to enhance care coordination and service delivery.

Qualifications:

• Hold a current RN license in New York State.
• Possess at least two years’ recent experience in a home care setting within the last ten years.
• Demonstrate proficiency conducting assessments similar to UAS, including OASIS or UAS-NY roles.
• Have experience caring for geriatric populations and managing chronic diseases at home.
• Capable of working independently in fast-paced, deadline-driven environments.
