Job Description
Job Summary:
The Case Manager RN in a hospital setting is responsible for coordinating patient care, facilitating care transitions, and ensuring appropriate utilization of resources throughout the acute care episode. The role involves collaboration with an interdisciplinary team to promote optimal health outcomes and prevent unnecessary readmissions.
Responsibilities:
- Lead and coordinate comprehensive care plans for hospital patients, ensuring timely progression and appropriate level of care.
- Review patient medical records, assess psychosocial and financial needs, and determine risk levels for readmission.
- Collaborate with physicians, social workers, and care team members to develop and modify discharge plans.
- Oversee and direct the activities of Case Management Assistants to facilitate patient transfers and care coordination.
- Monitor length of stay and use escalation pathways to address complex clinical and social issues, reducing delays and unnecessary admissions.
- Ensure compliance with regulatory requirements, including CMS and Joint Commission standards, and complete necessary documentation.
Qualifications:
- Hold a valid California RN license with at least 5 years of acute inpatient case management experience.
- Demonstrate knowledge of care coordination, utilization review, and hospital discharge processes.
- Possess BLS certification and Epic charting experience preferred.
- Have experience managing complex psychosocial issues and social determinants of health.
- Ability to work collaboratively in a multidisciplinary team and communicate effectively across diverse populations.
- Be available to work a 5/8 schedule, including every other weekend, with willingness to participate in lab work within 3-5 days of offer.
