Job Summary:

Seeking an experienced RN Case Manager for hospital inpatient acute care to coordinate patient care, facilitate transitions, and ensure optimal resource utilization. The role involves collaboration with interdisciplinary teams, physicians, and community resources to deliver high-quality, cost-effective care.

Responsibilities:

• Assess and review patient admission plans, gather comprehensive medical, psychosocial, and financial information.
• Develop and oversee individualized, cost-effective care coordination and transition plans.
• Review medical records to confirm ongoing compliance with level of care requirements.
• Coordinate discharge planning, including follow-up appointments and post-acute care arrangements.
• Collaborate with the interdisciplinary team to resolve barriers and prevent unnecessary readmissions.
• Maintain accurate documentation within electronic health records, ensuring regulatory compliance.

Qualifications:

• CA RN license with at least 2 years of inpatient acute care case management experience.
• Valid BLS certification and current AHA credentials.
• Demonstrated proficiency in Epic or similar EHR systems.
• Strong knowledge of CMS and Joint Commission standards, including Transitions of Care.
• Ability to work collaboratively across departments and with external community resources.
• Excellent communication, assessment, and critical thinking skills.
