Job Summary:

Provides clinical oversight and coordination for hospitalized patients, ensuring comprehensive care management, documentation, and timely reviews to optimize patient outcomes and facility efficiency.

Responsibilities:

• Conduct chart audits to assess compliance and quality standards.
• Facilitate meetings and reviews to discuss patient care plans.
• Coordinate with healthcare teams to ensure effective communication.
• Maintain accurate documentation using Epic and other clinical systems.
• Monitor travel and licensing compliance, including background checks.
• Assist with staff onboarding and training modules as needed.

Qualifications:

• Hold a valid California Registered Nurse license.
• Two years of acute hospital nursing experience.
• Demonstrate proficiency in Epic charting and documentation.
• Provide proof of AHA certifications at the time of submission.
• Possess reliable transportation within 50 miles if required.
• Ability to pass lab work screening within 3-5 days of onboarding.
