Job Summary:
Seeking a versatile Case Manager with acute care experience capable of working across multiple units as needed. This role requires cross-training in various departments to support patient care and coordination efforts.

Responsibilities:

• Coordinate patient care plans and facilitate communication among healthcare teams.
• Float between units as assigned to ensure appropriate patient management.
• Complete all clinical documentation accurately using Epic electronic health record system.
• Maintain compliance with travel, certification, and licensing requirements.
• Participate in orientation modules and ongoing training to stay current with policies.
• Adhere to background checks, including verification of work history, and report any prior background issues.

Qualifications:

• Hold an active RN license valid in the state.
• Possess BLS certification; AHA certifications required and must be in hand at submission.
• Have a minimum of 2 years of relevant acute care nursing experience, including case management.
• Demonstrate experience in Epic or similar EHR systems.
• Be flexible with variable shifts, including day and evening options, and work every other weekend.
• Be able to travel up to 50 miles; locals accepted at lower rates.
