Job Summary:
Provide comprehensive in-home nursing assessments for patients in a home health setting, ensuring accurate evaluation of health status, care needs, and service planning. Utilize clinical expertise and technology to support patient care and care coordination efforts.

Responsibilities:

• Perform initial in-home pre-evaluations, assessing health status, strengths, and preferences to develop personalized care plans.
• Review medical documentation and health referral forms pertinent to each case.
• Enter evaluation data into electronic systems and transmit as required to support enrollment and case documentation.
• Document concerns, conflicting information, and issues encountered during assessments.
• Communicate with consumers regarding health care options, support services, and waiver programs.
• Collaborate with interdisciplinary team members to enhance care coordination and service delivery.

Qualifications:

• Valid RN licensure in New York State.
• At least two (2) years of recent experience conducting assessments in a home setting within the last ten (10) years.
• Proven experience with UAS-NY assessments (role 40 or 45), especially in the last 365 days.
• Experience caring for geriatric populations and managing chronic diseases at home.
• Ability to work independently in a fast-paced environment, balancing multiple complex tasks.
