Physical Therapist
TACT STAFF is seeking a skilled Physical Therapist to join our team! As a physical therapist, you’ll be a vital member of a healthcare team, all working together to help patients with both acute and chronic conditions. This role is working in an outpatient setting.
Responsibilities:

· Perform comprehensive physical assessments to identify impairments, limitations, and functional abilities.

· Analyze assessment data to develop accurate diagnoses and individualized treatment plans.

· Treatment Planning and Implementation:

· Design and implement evidence-based, goal-oriented treatment plans tailored to meet the unique needs of each patient.

· Utilize a variety of therapeutic techniques, exercises, and modalities to promote optimal recovery and function.

· Educate patients and their families on home exercise programs and self-management strategies.

· Maintain accurate and up-to-date patient records, documenting treatment plans, progress notes, and discharge summaries in compliance with regulatory standards.

· Provide regular reports to the healthcare team on patient progress, setbacks, and adjustments to treatment plans.

· Communicate effectively with patients and their families, explaining treatment plans, goals, and expectations in a clear and empathetic manner.

· Stay current with advances in physical therapy practices and techniques through ongoing education and training.

· Actively participate in team meetings, case conferences, and professional development activities to enhance clinical skills and knowledge.

Requirements:

· Doctor of Physical Therapy (DPT) degree from an accredited program.

· New Jersey State licensure as a Physical Therapist.

· CPR certification.

· Strong clinical reasoning and problem-solving skills.

· Excellent interpersonal and communication skills.
