Job Summary:
The Physical Therapist is responsible for developing and implementing treatment plans to improve patient mobility and functionality within a Skilled Nursing Facility setting. They evaluate patient needs, administer therapy sessions, and monitor progress to facilitate recovery and enhance quality of life.

Responsibilities:

• Evaluate patients’ physical condition to determine appropriate therapy interventions.
• Design and implement individualized treatment plans aligned with patient goals and medical guidance.
• Administer therapeutic exercises, manual therapy, and modalities to improve mobility and strength.
• Monitor patient progress and adjust treatment strategies to achieve optimal outcomes.
• Document patient assessments, treatment plans, and progress notes accurately and promptly.
• Collaborate with interdisciplinary team members to coordinate care and ensure treatment efficacy.

Qualifications:

• Hold a valid physical therapy license and meeting state licensure requirements.
• Possess proven experience working with elderly or SNF populations.
• Demonstrate strong clinical assessment and problem-solving skills.
• Maintain current knowledge of physical therapy techniques and standards.
• Exhibit excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
