Job Summary:
The Case Manager RN will coordinate complex inpatient case management services across the healthcare system, primarily working virtually to support patients in the community. They may need to float between offices in Emeryville and San Francisco, but will mostly operate remotely.

Responsibilities:

• Collaborate with healthcare teams to develop and implement individualized care plans for patients.
• Monitor patient progress and adjust care strategies in accordance with clinical guidelines.
• Facilitate communication among patients, families, and interdisciplinary teams.
• Ensure compliance with all licensing, certification, and documentation requirements.
• Complete all required documentation, assessments, and reports within designated time frames.
• Support hospital and community-based care transitions to optimize patient outcomes.

Qualifications:

• Possess a valid California RN license with current Nursys verification within the last 2 weeks.
• Have at least 2 years of inpatient case management experience.
• Hold applicable AHA or ARC certifications with valid expiration dates.
• Demonstrate proficiency in managing complex cases and working independently in a virtual environment.
• Be willing to float between Emeryville and San Francisco offices as needed.
• Comply with all travel, holiday, and shift policies outlined by the facility.
