Job Summary:
Provides comprehensive physical therapy services to patients within the legal scope of practice, ensuring quality care and compliance with all regulatory requirements. Develops and implements individualized treatment plans to promote recovery, mobility, and independence.

Responsibilities:

• Conduct initial evaluations, progress notes, reassessments, and recertifications in accordance with Medicare, state, and agency standards.
• Develop, implement, and update patient-centered plans of care aligned with physician orders and patient needs.
• Deliver skilled interventions and maintenance therapy programs within the scope of practice.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides, ensuring adherence to licensing and practice acts.
• Educate patients and caregivers on home exercise routines, mobility strategies, and adaptive techniques to enhance safety and independence.
• Document all clinical interventions, communication, and care coordination promptly and accurately.

Qualifications:

• Graduate of an accredited physical therapist program approved by a recognized accrediting body.
• Currently licensed as a Physical Therapist in the state of practice.
• Minimum of two years of clinical experience preferred; home health experience is a plus.
• Strong clinical judgment with the ability to work independently across varied patient settings.
• Proficient in computer skills, with a willingness to learn EMR and OASIS documentation.
• Current CPR certification required.
