Job Summary:
Provide comprehensive occupational therapy services to residents in a post-acute care setting, promoting independence and improving quality of life through personalized intervention plans. Collaborate with a multidisciplinary team to deliver high-quality, patient-centered care in a supportive environment.

Responsibilities:

• Evaluate residents' needs and develop individualized treatment plans.
• Implement therapeutic interventions to enhance functional abilities, activities of daily living (ADLs), and safety.
• Collaborate with nursing and rehabilitation staff to coordinate care and optimize outcomes.
• Maintain accurate documentation in compliance with state and federal regulations.
• Educate residents and families on treatment goals and progress.

Qualifications:

• Hold a current California Occupational Therapist (OT) license.
• Demonstrate strong interpersonal and clinical skills.
• Show commitment to delivering high-quality patient care.
• Preferably have experience in skilled nursing or post-acute care settings, though not required.
• Exhibit a collaborative, team-oriented approach to rehabilitation.
