Job Summary:
Provides comprehensive in-home assessments for clients to determine health status, care needs, and support plans. Works independently to evaluate medical conditions, review documentation, and collaborate with care teams to ensure optimal service delivery.

Responsibilities:

• Conduct initial in-home pre-evaluations assessing health status, strengths, and care preferences.
• Review medical documentation and health referral forms relevant to each case.
• Document evaluation findings, concerns, and conflicting information accurately in electronic systems.
• Communicate options regarding healthcare supports, services, vendors, and waiver programs to consumers.
• Facilitate communication between the Call Center and clients for enrollment in managed care health plans.
• Collaborate with interdisciplinary team members to coordinate care and improve service delivery.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two years recent experience in a home health setting within the last 15 years.
• Demonstrated ability to perform comprehensive patient assessments and review medical documentation.
• Strong organizational, communication, and interpersonal skills.
• Ability to work independently in a fast-paced environment with multiple priorities.
• Proficiency in using technology for documentation and evaluation purposes.
