Job Summary:
Provides comprehensive nursing care to residents in a Long-Term Care or Skilled Nursing Facility setting. Ensures patient safety, promotes well-being, and collaborates with the healthcare team to deliver quality care.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and assess residents' health status, reporting significant changes promptly.
• Document patient care activities and maintain detailed medical records.
• Educate residents and family members on health management and care plans.
• Follow all facility policies and regulations to ensure compliance and safety.
• Collaborate with physicians, therapists, and staff to coordinate comprehensive care.

Qualifications:

• Valid RN license in the state of practice.
• Proven experience working in LTC/SNF or similar settings.
• Strong assessment and critical-thinking skills.
• Ability to work effectively within a team environment.
• Excellent communication and interpersonal skills.
• Commitment to infection control protocols, including COVID-19 vaccination or exemption documentation.
