Job Summary:
The RN - Nurse Assessor is responsible for conducting comprehensive health evaluations in patients' homes to support enrollment in managed care programs. The role involves evaluating health status, developing care plans, and coordinating with interdisciplinary teams to ensure quality patient care.

Responsibilities:

• Perform initial in-home assessments to evaluate patient health, strengths, and care needs.
• Review medical documentation and referral forms to inform assessments.
• Enter evaluation data into electronic systems and transmit as required.
• Document concerns, conflicting information, and other findings during assessments.
• Facilitate communication between the Call Center and consumers for enrollment in managed care plans.
• Collaborate with care teams to enhance care coordination and service delivery.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two (2) years recent experience in a home setting within the last ten (10) years.
• Experience conducting assessments such as UAS-NY, OASIS, or similar.
• Proven ability to perform in fast-paced, deadline-driven environments with excellent communication skills.
• Knowledge of state regulations, community resources, and care management processes.
• Strong organizational and interpersonal skills, with sensitivity to diverse populations.
