Job Summary:
Provide comprehensive in-home nursing assessments to evaluate patients’ health status, strengths, care needs, and preferences. Utilize technology and critical thinking skills to support individualized care planning and ensure seamless service delivery in a fast-paced environment.

Responsibilities:

• Conduct initial in-home pre-evaluations, including health assessments and care planning.
• Review medical documentation and health referral forms to inform evaluations.
• Input evaluation data into electronic forms and transmit data accurately.
• Document concerns, conflicting information, or issues identified during assessments.
• Communicate healthcare options, supports, and waiver options with consumers.
• Coordinate with the Call Center and multidisciplinary teams to facilitate enrollment and care continuity.

Qualifications:

• Hold current RN licensure in New York State.
• Possess a minimum of two years of recent experience conducting comprehensive assessments in a home setting.
• Demonstrate proficiency with technology and electronic documentation systems.
• Exhibit excellent communication, organizational, and interpersonal skills.
• Be able to work independently in a fast-paced, deadline-driven environment.
