Job Summary:
Provide expert physical therapy services to home health clients, focusing on improving patient mobility and function through individualized treatment plans and therapeutic interventions. Collaborate with multidisciplinary teams to develop comprehensive care strategies that meet patient needs.

Responsibilities:

• Evaluate patients’ physical health status and develop personalized treatment plans.
• Administer therapeutic modalities and use rehabilitation equipment, including splints, orthoses, prostheses, wheelchairs, and ADL equipment.
• Collaborate with healthcare team members to coordinate patient care and progress outcomes.
• Document patient assessments, treatment plans, and progress accurately in compliance with regulations, including proficiency with OASIS.
• Monitor and adjust therapy interventions based on patient response and clinical judgment.
• Educate patients and caregivers on exercises, equipment use, and safety protocols for home health care.

Qualifications:

• Valid MA Physical Therapist license and current BLS certification.
• Minimum of 2 years acute hospital-based nursing care and home health care experience.
• Must be Home Health Certified and proficient with OASIS documentation.
• Own reliable vehicle and include permanent address in profile.
• Ability to work independently with minimal supervision.
• Strong communication and patient education skills.
