Job Summary:

Provide comprehensive home health nursing care to patients in their residences, ensuring adherence to care plans and safety protocols. Collaborate with interdisciplinary teams to promote patient health and recovery.

Responsibilities:

• Assess patients' health conditions and develop individualized care plans.
• Administer medications and treatments following physician orders.
• Educate patients and caregivers on disease management and health maintenance.
• Document patient progress and update medical records accurately.
• Monitor and evaluate patient responses to interventions and modify care plans as needed.
• Coordinate with physicians and other healthcare professionals to ensure quality care delivery.

Qualifications:

• Valid Registered Nurse (RN) license in the state of practice.
• Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification from an approved vendor.
• Valid driver’s license and reliable automobile with current automobile insurance.
• Minimum age of 21 years or older.
• Demonstrated experience in home health nursing preferred.
• Strong communication and patient education skills.
