Job Summary:

Provide specialized physical therapy services to patients in a home health setting across various locations on the island. Deliver care that promotes recovery and improves patients' mobility and functional abilities.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic exercises and interventions to restore function.
• Monitor and document patient progress accurately and timely.
• Collaborate with care teams and communicate effectively with patients and families.
• Ensure compliance with healthcare regulations, including documentation standards.
• Manage a caseload of patients, scheduling visits to meet visit and visit-hour goals.

Qualifications:

• Hold an active Hawaii license or pending license with eTAO approval.
• Possess a minimum of 1 year of relevant home health or outpatient experience.
• Have Oasis experience preferred.
• Maintain current BLS certification from AHA.
• Have a valid driver’s license and personal transportation.
• Demonstrate strong communication and documentation skills.
