Job Summary:
The Occupational Therapist provides assessment and rehabilitation interventions in the home health setting to improve patient functional outcomes. The role involves care planning, patient education, and supervision of therapy services within a multidisciplinary team.

Responsibilities:

• Evaluate patients to develop individualized care plans addressing their needs in the home environment.
• Deliver direct occupational therapy services aligned with physician orders and evidence-based guidelines.
• Apply CMS OASIS scoring and collaborate with the Quality team to ensure accurate documentation and adjustments.
• Participate in multidisciplinary care conferences to establish and modify relevant care strategies.
• Document all services, adverse events, and communications thoroughly in the medical record.
• Supervise OT Assistants and oversee care provided by CNA staff when appropriate.

Qualifications:

• Hold a Bachelor’s or higher degree from an accredited Occupational Therapy program.
• Possess a valid OT license to practice in Montana.
• Maintain a current driver’s license and auto insurance.
• Bring at least two years of previous OT experience preferred.
• Demonstrate knowledge of Medicare regulations, reimbursement requirements, and ethical practice standards.
