Job Summary:
Seeking a dedicated Registered Nurse to provide in-home patient care within the Home Health setting. The ideal candidate will deliver compassionate nursing services to geriatric patients and coordinate care efficiently.

Responsibilities:

• Assess patients' health conditions and develop individualized care plans.
• Administer medications, treatments, and wound care as prescribed.
• Communicate effectively with patients, families, and healthcare teams.
• Document patient visits, progress, and any changes in health status accurately.
• Ensure compliance with safety standards, including BLS/CPR certification.
• Maintain up-to-date knowledge of Oasis documentation and home health protocols.

Qualifications:

• Hold a valid California RN license or have an active license application in progress.
• Minimum of 1 year of Home Health experience, particularly in geriatrics.
• Possess BLS/AHA certification and Oasis experience.
• Demonstrate strong communication skills and attention to detail.
• Valid driver’s license and personal transportation required.
• Willing to work weekends and flexible hours as confirmed during interview.
