Job Summary:
Provide home health care services to patients in their residences, supporting hygiene, daily living activities, and comfort needs. Collaborate with the healthcare team to deliver safe, compliant, and patient-centered care within the legal scope of practice.

Responsibilities:

• Administer routine patient care and support to meet hygiene and daily living needs according to established standards.
• Support patients in activities of daily living, including mobility, personal hygiene, and comfort measures.
• Perform specific procedures and assist in patient education as required.
• Exercise sound judgment and clinical skills to support the patient's individualized plan of care.
• Adhere to all local, state, and federal regulations to ensure patient privacy and safety.
• Document care activities accurately and maintain compliance with policies and procedures.

Qualifications:

• Valid Home Health Aide (HHA) and Certified Nursing Assistant (CNA) licenses required.
• Current BLS (Basic Life Support) healthcare certification.
• Valid Driver’s License with auto insurance coverage.
• Minimum of 2 years of relevant home health experience preferred.
• Ability to provide competent patient care with strong communication skills.
