Job Summary:

We are seeking a dedicated Physical Therapy Assistant to provide home health therapy services, including home visits and patient follow-ups. The ideal candidate will support patient rehabilitation goals while ensuring quality care and compliance with industry standards.

Responsibilities:

• Deliver skilled physical therapy services to patients in their homes, following physician’s plans of care.
• Conduct initial assessments, treatment planning, and follow-up visits to monitor patient progress.
• Document clinical activities accurately in the EMR system within 24 hours of each visit.
• Communicate with supervising therapists, case managers, and patients to coordinate care effectively.
• Maintain reliable transportation and ensure all documentation, travel, and activity points meet agency requirements.
• Adhere to patient scheduling, productivity expectations, and on-call schedules as required.

Qualifications:

• Hold an active and valid state license as a Physical Therapy Assistant.
• Have at least 1 full year of recent home health experience.
• Possess current CPR and BLS certifications.
• Demonstrate proficiency with home health EMR systems, preferably HomeCare HomeBase (HCHB).
• Be willing to travel up to 75 miles from the home clinic and accept patient schedules regardless of distance.
• Maintain reliable transportation, driver’s license, and agency-required liability insurance.
