Job Description
Job Summary:
Home Health SLP provides speech-language evaluation and treatment in patients’ homes to support communication, swallowing, and functional outcomes. This role delivers patient-centered therapy services Monday through Friday while adhering to clinical and documentation standards.
Responsibilities:
- Evaluate patients’ speech, language, cognitive-communication, and swallowing needs using appropriate screening and assessment tools.
- Develop and implement individualized therapy plans with measurable goals and evidence-based interventions.
- Deliver skilled speech-language pathology services and provide caregiver education to reinforce therapy between visits.
- Document clinical findings, progress notes, and outcomes in accordance with applicable standards and timelines.
- Coordinate ongoing care by communicating with the interdisciplinary team and following physician and plan-of-care directions.
- Maintain safety compliance through current CPR/BLS and infection control practices during home visits.
Qualifications: - Hold a valid Speech-Language Pathologist license in the state of practice.
- Possess ASHA Certificate of Clinical Competence (CCC) (preferred).
- Maintain current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor per policy requirements.
- Possess a valid driver’s license and meet minimum age requirements per policy.
- Demonstrate strong clinical documentation skills and the ability to manage home-based caseloads independently.
- Show proficiency delivering education and coaching to patients and caregivers.
