Job Summary:


Home Health SLP provides speech-language evaluation and treatment in patients’ homes to support communication, swallowing, and functional outcomes. This role delivers patient-centered therapy services Monday through Friday while adhering to clinical and documentation standards.



Responsibilities:

• Evaluate patients’ speech, language, cognitive-communication, and swallowing needs using appropriate screening and assessment tools.
  
• Develop and implement individualized therapy plans with measurable goals and evidence-based interventions.
  
• Deliver skilled speech-language pathology services and provide caregiver education to reinforce therapy between visits.
  
• Document clinical findings, progress notes, and outcomes in accordance with applicable standards and timelines.
  
• Coordinate ongoing care by communicating with the interdisciplinary team and following physician and plan-of-care directions.
  
• Maintain safety compliance through current CPR/BLS and infection control practices during home visits.
  
  
  
  Qualifications:
  
• Hold a valid Speech-Language Pathologist license in the state of practice.
  
• Possess ASHA Certificate of Clinical Competence (CCC) (preferred).
  
• Maintain current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor per policy requirements.
  
• Possess a valid driver’s license and meet minimum age requirements per policy.
  
• Demonstrate strong clinical documentation skills and the ability to manage home-based caseloads independently.
  
• Show proficiency delivering education and coaching to patients and caregivers.
