Job Summary:

Provides comprehensive home health nursing care to patients based on physicians’ orders, employing the nursing process to promote recovery and well-being. Oversees the delivery of patient care, ensuring compliance with agency policies and professional standards.

Responsibilities:

• Coordinate and provide home health nursing visits to patients within designated territory.
• Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses.
• Document patient assessments, interventions, and outcomes accurately within the EMR system.
• Manage scheduling to meet patient needs while adhering to productivity and mileage expectations.
• Maintain current licensure, certifications, and proficiency in required clinical documentation systems.
• Comply with on-call and weekend rotation requirements as scheduled.

Qualifications:

• Hold a current valid RN license in the state of practice.
• Possess at least one year of recent adult home health or hospice experience.
• Demonstrate proficiency in HomeCare HomeBase (HCHB) and OASIS (for RNs).
• Have reliable transportation, a valid driver’s license, and valid auto insurance.
• Be willing to travel up to 75 miles from the home clinic.
• Be capable of working independently and managing a variable caseload efficiently.
