Job Summary:

The Physical Therapy Assistant will support patient recovery by implementing prescribed therapy plans in home health settings. The role involves visiting patients' homes, documenting progress, and collaborating with supervising therapists to ensure optimal care.

Responsibilities:

• Assist licensed therapists in delivering therapeutic interventions and exercises.
• Conduct home visits to evaluate patient progress and modify treatment as directed.
• Document patient sessions accurately within the EMR system in a timely manner.
• Travel to multiple patient locations within a designated territory, following scheduled appointments.
• Communicate effectively with patients and caregivers to ensure understanding of therapy goals.
• Maintain current licensure and certification requirements, including CPR and BLS.

Qualifications:

• Minimum of one year of recent experience as a Physical Therapy Assistant in home health or similar settings.
• Valid PTA license in the state of employment with no pending license issues.
• Proficiency in using EMR systems, specifically HCHB is preferred.
• Reliable transportation with current driver’s license and auto insurance.
• Ability to travel up to 75 miles from the home clinic and handle a flexible schedule.
• Good communication and documentation skills, and ability to work independently.
