Job Summary:
Provides specialized occupational therapy services to patients in a home care and hospice setting, aiming to enhance their independence and safety through personalized in-home interventions.

Responsibilities:

• Assess patients' functional abilities and develop tailored treatment plans.
• Deliver in-home therapy sessions to improve patients' physical, motor, and cognitive skills.
• Collaborate with a multidisciplinary team to coordinate patient care and progress.
• Document patient progress and update care plans in compliance with agency standards.
• Educate patients and caregivers on therapeutic exercises and safety measures.

Qualifications:

• Hold a valid Vermont occupational therapy license.
• Possess a valid driver’s license and reliable personal vehicle.
• Demonstrate experience in a home health care environment (preferred).
• Exhibit strong independent work skills and collaborative abilities.
• Maintain current CPR certification (if required).
