Job Summary:
Provide comprehensive nursing care to inpatient rehabilitation patients during night shift hours, focusing on assessment, planning, and implementing individualized care plans. Collaborate with the healthcare team to ensure quality patient outcomes in a high neuro population setting.

Responsibilities:

• Assess patients’ mental, physical, and spiritual needs upon admission and throughout the shift.
• Develop and execute personalized care plans in coordination with the multidisciplinary team.
• Monitor patient conditions, recognizing and reporting any changes using clinical judgment.
• Educate patients and designated responsible persons on treatment plans, procedures, and discharge instructions.
• Document medical records accurately and maintain compliance with healthcare standards.
• Contribute to a positive, conflict-free work environment through professional communication and teamwork.

Qualifications:

• Hold a current Illinois Registered Nurse license.
• Possess a valid CPR/BLS certification.
• Demonstrate experience or familiarity with neuro patient populations, with stroke certification being a plus.
• Exhibit strong clinical judgment and effective communication skills.
• Show flexibility to work weekends and willingness to accommodate schedule needs.
