Job Summary:
Provides comprehensive home nursing care by coordinating and delivering services in accordance with physicians' orders and agency policies. Responsible for supervising and evaluating Home Health Aides and LPNs/LVNs, ensuring high-quality patient care through the nursing process.

Responsibilities:

• Coordinate and execute home health visits, including Start of Care, wound care, and follow-up assessments.
• Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses to maintain quality standards.
• Document patient care accurately within required timeframes using HomeCare HomeBase and Oasis when applicable.
• Travel up to 75 miles from the home clinic, conducting patient visits with reliable transportation.
• Participate in periodic office meetings and on-call rotations, including weekends, as required.
• Maintain current licensure, certifications, and proficiency in EMR systems to ensure compliance.

Qualifications:

• Hold an active RN license in the assigned state or territory.
• Possess at least 1 year of recent adult home health or hospice experience.
• Demonstrate proficiency in HomeCare HomeBase; Oasis experience preferred for RNs.
• Have a valid driver’s license, reliable vehicle, and agency-required liability insurance.
• Be willing to travel up to 75 miles, accommodating different geographic zones.
• Be able to work independently, managing a caseload of 5-8 patients daily.
