Job Summary:
Provides professional nursing care to patients in a home health setting, following physicians' orders and agency policies. Coordinates and supervises home health aides and licensed practical nurses to ensure comprehensive patient care.

Responsibilities:

• Administer and coordinate skilled nursing services to patients in their homes.
• Supervise and evaluate the performance of Home Health Aides and LPNs/LVNs.
• Document patient assessments, care plans, and ongoing progress within mandated timeframes.
• Travel up to 75 miles per day to deliver care across designated territory.
• Maintain compliance with all agency protocols, including proficiency in HomeCare HomeBase and Oasis (for RNs).
• Participate in team meetings and ensure seamless communication with healthcare providers.

Qualifications:

• Hold a valid RN license in the state of assignment.
• Possess a minimum of 1-year current adult home health or hospice experience.
• Demonstrate proficiency in Oasis documentation (for RNs).
• Have reliable transportation, a valid driver’s license, and current auto liability insurance.
• Be able to travel up to 75 miles from the home clinic and work flexible hours including one weekend and one on-call week per month.
• Experience with HomeCare HomeBase (HCHB) EMR software.
