Job Summary:
Provides specialized wound care nursing services, assessing and managing patients with complex wounds. Collaborates with healthcare team members to develop and implement effective care plans, ensuring optimal patient outcomes and safety.

Responsibilities:

• Conduct thorough wound assessments and develop individualized treatment plans based on patient needs and condition changes.
• Collaborate with physicians, nurses, and other healthcare providers to coordinate patient care and adjust plans as necessary.
• Deliver compassionate, evidence-based wound care interventions while maintaining a safe and supportive environment.
• Communicate care plans clearly to patients, families, and team members, ensuring understanding through teach-back and educational methods.
• Monitor, evaluate, and document patient progress, adjusting interventions to improve healing and prevent complications.
• Ensure compliance with hospital policies, regulations, and safeguarding patient confidentiality at all times.

Qualifications:

• Hold a valid CA RN license and current BLS certification.
• Minimum of 2 years of clinical nursing experience, preferably including wound care.
• Demonstrate proficient EPIC electronic health record system experience or similar charting software.
• Wound care certification preferred; relevant clinical expertise required.
• Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
• Clear communication skills and attention to detail.
