Job Summary:
Provides skilled physical therapy assistance to patients under the supervision of a licensed physical therapist. Focuses on implementing treatment plans to improve patient mobility and function in a home health setting.

Responsibilities:

• Assist with the assessment and treatment of patients according to established protocols.
• Implement therapeutic interventions prescribed by the physical therapist.
• Monitor and document patient progress and report to the supervising therapist.
• Educate patients and caregivers on exercises and activities to promote recovery.
• Maintain clinical documentation and ensure compliance with health standards.
• Adhere to all safety and infection control policies.

Qualifications:

• Valid SC PTA license required; must obtain prior to start date.
• Minimum of 1-2 years’ experience in home health setting preferred.
• Demonstrate competency in therapeutic techniques and patient care.
• Recent EPIC experience preferred but not mandatory.
• Ability to pass initial orientation tests, including Cardiac Dysrhythmia and Moderate Sedation if applicable.
• Strong communication and documentation skills.
