Job Summary:

Seeking a dedicated Physical Therapist to provide home health services in the Grass Valley and Yuba County area. The ideal candidate will evaluate, plan, and implement individualized treatment plans to help patients improve their mobility and overall well-being.

Responsibilities:

• Assess patient condition and develop personalized treatment plans tailored to their needs.
• Deliver hands-on therapy and interventions to improve patient mobility and function.
• Document patient progress and update treatment plans accordingly.
• Collaborate with patients, families, and healthcare teams to ensure optimal care.
• Maintain compliance with all clinical and safety protocols.
• Participate in training and continuing education as needed.

Qualifications:

• Hold a valid Doctor of Physical Therapy (DPT) credential, obtained past 2016.
• Possess at least one year of home health experience or willingness to be trained.
• Current BLS certification.
• Must be 21 years or older.
• Demonstrate excellent communication and patient care skills.
• Be able to work independently in a home health setting.
