Job Summary:
Provide comprehensive inpatient case management services to ensure optimal patient outcomes and efficient healthcare delivery. Collaborate with multidisciplinary teams to coordinate care and facilitate smooth patient transitions.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Monitor patient progress and adjust care plans accordingly.
• Collaborate with physicians, social workers, and other healthcare professionals.
• Educate patients and families about care options and discharge instructions.
• Ensure compliance with hospital policies and regulatory standards.

Qualifications:

• Hold a valid RN license in the state of practice.
• Demonstrate experience in hospital-based case management or related clinical roles.
• Possess strong critical thinking and clinical assessment skills.
• Exhibit excellent communication and interpersonal skills.
• Maintain current CPR/BLS certification.
