Job Summary:
Provides in-home occupational therapy services to patients, enhancing their independence and safety through tailored interventions. Works independently while collaborating with a multidisciplinary team to deliver exceptional patient care.

Responsibilities:

• Assess patients’ functional abilities and develop individualized treatment plans.
• Deliver therapeutic interventions aimed at improving daily living skills.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Educate patients and caregivers on therapeutic techniques and safety measures.
• Adhere to all state licensure requirements and safety protocols.
• Collaborate with team members to coordinate patient care and ensure optimal outcomes.

Qualifications:

• Possess current Vermont licensure as an Occupational Therapist.
• Hold a valid driver’s license and reliable vehicle for home visits.
• Demonstrate experience in a home care or community-based setting.
• Exhibit strong communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Maintain valid CPR certification (preferred).
