Job Summary:
Seeking a skilled Speech Language Pathologist (SLP) to evaluate, diagnose, and treat patients with communication, cognitive, and swallowing disorders. The ideal candidate will develop individualized treatment plans and collaborate with healthcare teams to optimize patient outcomes.

Responsibilities:

• Evaluate patients using standardized assessments and evidence-based practices to determine communication, cognitive, and swallowing impairments.
• Develop and implement comprehensive treatment plans tailored to each patient's needs and goals.
• Monitor patient progress, adjust therapies accordingly, and document advancements in compliance with regulatory standards.
• Provide consultation and counseling to patients, families, caregivers, and other providers regarding speech and language disorders.
• Communicate effectively with supervisors and interdisciplinary team members on patient status, barriers, and treatment strategies.
• Maintain accurate records, complete discharge summaries, and ensure documentation meets all clinical and regulatory requirements.

Qualifications:

• Hold active state licensure as a Speech-Language Pathologist.
• Possess a CCC certification from ASHA.
• Minimum of 1 year of SLP experience; new graduates with Level II experience are considered.
• Prior experience in LTC or SNF settings required.
• Demonstrate the ability to supervise and support CFY or student SLPs.
• Comply with ASHA and state practice acts, ethics, and professional conduct standards.
