Job Summary:
Provides comprehensive patient care coordination within a home health setting, focusing on complex cases and high-utilizer patients. Acts as a liaison among patients, families, healthcare providers, and external entities to ensure optimal health outcomes and compliance with regulatory standards.

Responsibilities:

• Lead the development, implementation, and ongoing revision of case management plans tailored to individual patient needs.
• Collaborate with multidisciplinary teams to coordinate care, monitor progress, and ensure timely delivery of services.
• Communicate effectively with physicians regarding resource utilization, treatment plans, and discharge planning.
• Manage transfers to tertiary centers, ensuring seamless continuity of care and proper documentation.
• Collect and oversee data necessary for regulatory and accreditation compliance.
• Conduct reviews of frequent emergency department visits to assess and improve patient management strategies.

Qualifications:

• Possess at least 2 years of Medicare-certified home health agency experience.
• Demonstrate experience with travel for home health visits.
• Hold an active Hawaii nursing license and valid driver's license with insurance.
• Maintain current COVID and flu vaccinations, with no declinations.
• Have familiarity with Homecare Home Base and OASIS documentation protocols.
• Preferably possess case management experience and a bachelor's degree in a related field.
