Job Summary:
Provide specialized nursing care to patients requiring case management services within an acute hospital setting. This role involves coordinating discharge plans, conducting chart reviews, and ensuring seamless patient transitions.

Responsibilities:

• Manage complex patient case loads by developing and implementing individualized care plans.
• Review medical charts and perform patient screenings to identify care needs.
• Coordinate discharge planning and facilitate communication among multidisciplinary teams.
• Identify and mitigate barriers to patient care and safe discharge.
• Utilize electronic medical records system (Meditech, Midas) efficiently to document assessments and plans.

Qualifications:

• Hold an active registered nurse (RN) license with a minimum of 2 years of acute hospital case management experience.
• Demonstrate proficiency in discharge planning, care coordination, and chart review.
• Possess effective communication and organizational skills to manage multitasking.
• Experience with EMR systems such as Meditech and Midas is required.
• Ability to work collaboratively in a fast-paced environment, with a focus on patient-centered care.
