Job Summary:

Seeking a dedicated and skilled Physical Therapist (PT) to provide comprehensive rehabilitative care in a Skilled Nursing Facility. The PT will evaluate patients, develop personalized treatment plans, and deliver therapies aimed at improving mobility, strength, and independence.

Responsibilities:

• Conduct detailed assessments of patients’ physical abilities, movement restrictions, and pain levels.
• Design and implement individualized treatment plans based on evaluations, medical history, and patient objectives.
• Administer evidence-based physical therapy interventions to enhance strength, coordination, balance, and mobility.
• Collaborate with healthcare team members to ensure coordinated, patient-centered care.
• Track patient progress, modify therapy plans accordingly, and maintain accurate documentation in compliance with regulations.
• Educate patients and caregivers on mobility techniques, exercise routines, and equipment use.

Qualifications:

• Graduate of an accredited Physical Therapy program.
• Valid state license to practice as a Physical Therapist.
• CPR certification (preferred or required per facility policy).
• Prior experience in a SNF or geriatric rehabilitative setting preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to work independently and within a multidisciplinary team.
